Male Pattern Baldness: Understanding the Best Treatment Options Available

Hair loss is one of the most common concerns among men, affecting confidence, self-image and quality of life. Male pattern baldness, also called male androgenetic alopecia, is driven largely by genetics and sensitivity to a hormone called dihydrotestosterone (DHT). Research suggests that approximately 50% of men experience some degree of androgenetic alopecia by age 50.

A Canadian-led study recently examined available treatment options to determine which medications may provide the greatest improvement in hair growth, comparing minoxidil (oral and topical formulations), finasteride, and dutasteride. Researchers analyzed 23 clinical studies involving men with androgenetic alopecia and compared changes in total hair count and terminal hair count after 24 and 48 weeks of treatment.

What Did the Study Find?

Dutasteride showed the greatest increase in total hair count at 24 weeks. Oral dutasteride 0.5 mg daily produced the largest improvement in total hair count at 24 weeks — more effective than finasteride 1 mg daily, oral minoxidil 5 mg daily, oral minoxidil 0.25 mg daily, and topical minoxidil solutions. Dutasteride works by blocking the enzyme responsible for converting testosterone into DHT. Since DHT contributes to shrinking hair follicles in male pattern baldness, reducing DHT levels may help slow hair loss.

Oral minoxidil showed strong results for terminal hair growth. For terminal hair count at 24 weeks, oral minoxidil 5 mg daily demonstrated the greatest increase. Terminal hairs are thicker, mature hairs that contribute most to visible hair density. However, oral minoxidil is considered an off-label treatment for hair loss and requires careful assessment because it was originally developed as a blood pressure medication.

Finasteride demonstrated strong long-term effectiveness. At 48 weeks, finasteride 5 mg daily showed the greatest increase in total hair count, and finasteride 1 mg daily showed the greatest increase in terminal hair count. Finasteride is one of the most established medications for male pattern baldness and is Health Canada-approved for this indication.

While research helps compare effectiveness, choosing a hair loss treatment is not only about which medication ranks highest. Healthcare providers must consider possible side effects and each patient’s health history.

Possible side effects. Finasteride and dutasteride can affect DHT levels and may be associated with sexual side effects, mood changes, and other hormone-related effects. Dutasteride may appear more effective in studies, but its long-term safety profile for hair loss is less established compared with finasteride.

Patient health history. Treatment selection may depend on age, other medications, cardiovascular history, blood pressure, personal preferences, and risk tolerance.
