  • Click here to navigate to the prescription Refill form
  • Fill out the form with your information
  • Submit the form by pressing the “Order” button on the bottom of the form
  • Congratulations! you submitted your very ONLINE order!

Login Instructions

  • Click here to navigate to the login portal
  • Enter your Card number and Pin provided to your by the Pharmacist
  • You can now check on your current prescriptions and enter a new one
  • Click “Order” to submit your new prescription or a prescription refill
  • Congratulations! you submitted your ONLINE order from your personal account!
