A Needle-Free Future for Anaphylaxis Care: Introducing Neffy

For individuals and families living with severe, life-threatening allergies, the directive has always been clear: carry your epinephrine auto-injector everywhere, and do not hesitate to use it at the first sign of anaphylaxis. Yet despite decades of evidence proving that early epinephrine administration saves lives, clinical data and pharmacy practice reveal a persistent challenge: fear of needles, accidental needle-stick injuries, and the sheer bulk of traditional auto-injectors frequently cause patients or caregivers to hesitate during a medical emergency.

That paradigm is about to change. Health Canada has approved Neffy (epinephrine nasal spray), the first and only needle-free emergency treatment for anaphylaxis. Licensed in Canada by ALK-Abello Pharmaceuticals, Neffy is anticipated to be commercially available in pharmacies starting the first week of August 2026.

Understanding the Therapeutic Mechanism

Anaphylaxis is a rapid, systemic Type I allergic reaction. Upon exposure to an allergen — whether food, insect stings, medications, or exercise — the sudden cross-linking of immunoglobulin E (IgE) on mast cells and basophils triggers massive cellular degranulation. This releases potent inflammatory mediators, resulting in sudden vasodilation, increased vascular permeability, and bronchial smooth muscle constriction.

Epinephrine remains the foundational first-line treatment, acting as a nonselective agonist of both alpha- and beta-adrenergic receptors. Neffy delivers a 2 mg dose of epinephrine in a single 0.1 mL nasal spray, which achieves rapid systemic absorption through the highly vascularized nasal mucosa.

• Alpha-adrenergic receptor activation induces vasoconstriction, effectively reversing the dangerous systemic vasodilation and capillary leakage that lead to hypotension, vascular collapse, and angioedema (swelling).
• Beta-adrenergic receptor activation relaxes bronchial smooth muscle tissue to alleviate wheezing, airway obstruction, and respiratory distress, while concurrently suppressing the further release of histamine and leukotrienes.

Clinical Indications and Patient Selection

Neffy 2 mg is authorized for the emergency treatment of acute anaphylaxis in adults and pediatric patients weighing 30 kg or more (approximately 66 pounds).

Health Canada has not currently authorized Neffy for patients weighing less than 30 kg. For young children below this weight threshold, standard low-dose epinephrine auto-injectors (0.15 mg or 0.1 mg) remain the clinical standard of care.

Crucial Protocols for Administration and Dosage

To ensure maximum bioavailability and therapeutic efficacy, the physical technique used to administer Neffy is highly specific:

• Strict two-device carry rule. Just like traditional auto-injectors, patients must have immediate access to two Neffy nasal sprays at all times. Up to 15% of anaphylactic episodes in Canada exhibit a biphasic pattern, where severe symptoms recur hours after initial resolution without further allergen exposure.
• The single-nostril / same-nostril rule. The initial dose is one single spray (2 mg) activated into one nostril. If clinical symptoms do not improve, or if they deteriorate after the initial dose, a second dose — using a brand-new, second Neffy device — must be administered in the exact same nostril, starting five minutes after the first administration.
• Physical alignment and nozzle positioning. The nozzle must be inserted fully into the nostril and held straight into the nose. It must not be angled toward the inner nasal septum or the outer nasal wall, as misdirection can cause the solution to run or be lost on the tissue, reducing systemic absorption.
• No sniffing. Unlike conventional therapeutic allergy nasal sprays, the patient must avoid sniffing during or immediately after activation. Sniffing draws the medication past the nasal mucosa and down into the throat, preventing optimal systemic absorption.
• Anatomical coordination for delivery. The product monograph outlines distinct hand-coordination instructions depending on who is delivering the medication, to prevent accidental misdirection:
  • Self-administration: use the right hand to administer into the right nostril; use the left hand to administer into the left nostril.
  • Caregiver administration: use the right hand to administer into the patient’s left nostril; use the left hand to administer into the patient’s right nostril.

Safety, Tolerability, and Storage Profiles

Neffy boasts a robust safety profile. Clinical trials reviewed by Canada’s Drug Agency demonstrated excellent systemic tolerability with zero serious adverse events reported.

• Common local side effects are primarily noted after a second dose. These include throat irritation (18.8%), headache (17.6%), nasal discomfort (12.9%), and a transient feeling of being jittery or anxious (10.6%).
• Environmental resilience and storage. Neffy offers superior storage flexibility compared to traditional auto-injectors, which are highly sensitive to extreme temperatures. It is stored at room temperature (15 degrees C to 30 degrees C) for up to 30 months. However, excursions up to 50 degrees C are permitted, making it far safer to carry during hot summer months. While it should not be intentionally frozen, if it is accidentally exposed to freezing temperatures, it remains fully usable once completely thawed.
