Statins May Lower Dementia and Heart Attack Risk in Older Adults, New Studies Find

Statins are already one of the most widely prescribed medications for lowering cholesterol and reducing the risk of heart attack and stroke. Two newly published studies suggest their benefits for older adults may go further than previously understood.

Fewer Heart Attacks and Strokes in Healthy Adults Over 70

Until recently, it wasn’t clear whether starting a statin later in life offered real benefit to otherwise healthy older adults who had never had a major cardiovascular event. A large Australian trial, led by Monash University and published in the New England Journal of Medicine, set out to answer that question directly.

The trial followed almost 10,000 independently living adults aged over 70, randomly assigning them to either a daily statin or a placebo and following them for nearly six years. Adults taking the statin were 30% less likely to have a first major cardiovascular event, such as a heart attack or stroke, and the rate of serious side effects was low. Researchers estimated that treating 37 older adults with a statin for six years would prevent one major cardiovascular event.

The trial also found no evidence that statins increased dementia risk, addressing a concern some patients raise when starting or continuing the medication.

A Possible Link to Lower Dementia Risk

A separate study out of Copenhagen University Hospital, published in The Lancet Regional Health Europe, followed more than 132,000 people with type 2 diabetes for an average of seven years. People who started a statin within a year of their diabetes diagnosis had a 15% lower risk of dementia than those who didn’t start one. Starting a statin between one and five years after diagnosis was linked to a 10% lower risk.

Researchers aren’t yet certain why statins might help protect the brain, but one leading theory involves LDL (“bad”) cholesterol: elevated LDL can contribute to fatty deposits narrowing the blood vessels that supply the brain, which reduces blood flow and raises the risk of stroke and tissue damage over time. Because this particular study was observational rather than a randomized trial, it can’t prove statins directly caused the lower dementia risk, but it adds to a growing body of evidence connecting cardiovascular health to brain health. Should You Be Taking a Statin?

These findings don’t mean every adult over 70 should automatically start a statin. Whether one is appropriate still depends on your overall cardiovascular risk, general health, and the other medications you’re taking. Statins are generally prescribed for people who already have cardiovascular disease or are considered at increased risk of a heart attack or stroke, and that clinical judgment hasn’t changed, what has changed is the evidence supporting statins as a reasonable option for more healthy older adults than previously assumed.
