But one ingredient found in many of these products—dextromethorphan (DXM)—is increasingly being misused by teens and young adults in search of a “cheap high.” This dangerous practice, often referred to as robotripping, is far from safe and can cause serious harm.
What is DXM?
DXM is an antitussive (cough suppressant) that has been widely used for decades. At normal doses, it safely helps relieve cough. But when taken in very high amounts, DXM can alter brain chemistry and produce hallucinogenic effects similar to drugs like ketamine or PCP.
Why is DXM Abuse a Concern?
- Accessibility: Some DXM-containing products are available without restrictions in Canada, making it easy for youth to obtain.
- Adolescent use: Surveys show that up to 10% of Ontario high school students report using DXM in ways other than intended.
- Severe risks: High doses can lead to confusion, hallucinations, irregular heartbeat, liver damage (when combined with acetaminophen), seizures, or even death.
- Unseen danger: Many DXM-containing products also include decongestants, pain relievers, or antihistamines. At high doses, these added ingredients multiply the risks.
Warning Signs of Misuse
Pharmacists and caregivers should be alert for:
- Young patients repeatedly purchasing large bottles of cough medicine.
- Signs of agitation, confusion, or disheveled appearance.
- Requests for multiple products containing DXM without clear medical need.
What Can Pharmacists Do?
Pharmacists play a frontline role in protecting the community by:
- Monitoring purchases of DXM-containing medications.
- Limiting quantities when a request seems unusual.
- Educating patients and families about the dangers of misuse.
- Referring to medical care if misuse or dependency is suspected.
Protecting Our Community
Several U.S. states have already banned the sale of DXM-containing products to individuals under 18. In Canada, some products remain unscheduled and available outside pharmacies, which raises concern. Until stronger regulations are in place, community pharmacies must remain vigilant.
