In 2025, Hypertension Canada released a new adult hypertension guideline specifically designed for primary care providers — including family physicians, nurse practitioners, and community pharmacists.

Lower Blood Pressure Threshold: Earlier Identification
Under the new guideline:
- A blood pressure (BP) of ≥ 130 / ≥ 80 mmHg is now considered elevated
- Previously, higher thresholds were used for diagnosis
This shift is based on strong evidence showing that cardiovascular risk increases even at these lower levels. Patients with BP in the 130–139 systolic range have higher rates of:
- Heart attack
- Stroke
- Heart failure
- Cardiovascular death
Earlier identification allows earlier intervention — particularly important for patients with diabetes, chronic kidney disease, or existing cardiovascular disease.
Diagnosis Still Requires Confirmation
To avoid overdiagnosis:
- BP must be measured using a validated automated (oscillometric) device
- Patients should rest 5 minutes before measurement
- At least 3 readings, 1 minute apart
- Out-of-office confirmation (home or ambulatory BP monitoring) is recommended

When Do We Start Medication?
Pharmacotherapy is recommended for:
- BP ≥ 140/90 mmHg (general population)
- SBP ≥ 130 mmHg if high cardiovascular risk:
- Established cardiovascular disease
- Diabetes
- Chronic kidney disease
- Age ≥ 75
- Framingham Risk Score ≥ 20%
New Target: Systolic BP < 130 mmHg
For most adults with hypertension, the recommended goal is:
Systolic BP < 130 mmHg (if well tolerated)
This recommendation is informed in part by data from the landmark SPRINT trial, which showed reduced cardiovascular events with more intensive BP control.
However, aggressive lowering increases risk of:
- Hypotension
- Falls
- Electrolyte imbalance
- Acute kidney injury
In frail patients or those at fall risk, the goal becomes:
“As low as reasonably achievable.”

Major Change: Start with Two Medications
The biggest practice shift:
➡ Initial low-dose combination therapy is now preferred
➡ Ideally as a single-pill combination (SPC)
First-line classes:
- ACE inhibitors (ACEIs)
- Angiotensin receptor blockers (ARBs)
- Thiazide or thiazide-like diuretics
- Long-acting dihydropyridine calcium channel blockers (DHP CCBs)
Beta-blockers are no longer first-line unless there is another indication (e.g., recent MI, heart failure with reduced EF, atrial fibrillation).
Why combination therapy?
- Faster BP control
- Better cardiovascular outcomes
- Improved adherence with single-pill combinations
Preferred Single-Pill Options
The guideline highlights:
- Irbesartan + hydrochlorothiazide
- ACEI (e.g., ramipril) + hydrochlorothiazide
ARBs may be better tolerated than ACEIs (less cough, lower angioedema risk).

Resistant Hypertension
If BP remains elevated on:
